三维重建下3D打印技术在下颌骨缺损修复中的应用评价

Application of 3D printing technology under three-dimensional reconstruction in mandibular reconstruction

摘要: 目的: 探讨三维重建下3D打印技术在下颌骨缺损修复手术中的应用效果。方法: 选择在南京医科大学附属口腔医院行下颌骨缺损修复重建的84例患者作为研究对象,根据治疗方式不同分为3D组(42例)和对照组(42例),对照组行常规手术,3D组采用数字三维重建3D打印技术辅助手术。记录2组的手术情况、手术并发症发生率及面容、咬合关系恢复情况等。采用SPSS 23.0软件包对数据进行统计学处理。结果: 3D组患者的手术时间比对照组短,术中出血量比对照组少(P<0.05)。3D组患者面容及咬合关系的恢复优良率(95.24%)比对照组(78.57%)显著提高(P<0.05)。与对照组相比,3D组手术前、后下颌骨各位点的移动距离均显著减小(P<0.05)。2组术后咀嚼功能、发音情况恢复满意度评分接近(P>0.05),但与对照组相比,3D组对外貌恢复的满意度评分更高(P<0.05)。结论: 在下颌骨缺损修复重建手术中,采用三维重建3D打印技术能够减少术中出血,缩短手术时间,外形恢复良好,患者满意度高。

Abstract: PURPOSE: To explore the application value of 3D printing technology under three-dimensional reconstruction in mandibular reconstruction. METHODS: Eighty-four patients with mandibular defect reconstruction were divided into two groups by different operation methods: 3D group(n=42) and control group(n=42). Patients in the control group underwent routine operation, while patients in the experimental(3D) group underwent three-dimensional reconstruction with 3D printing technology. The operation conditions, incidence of complications, recovery of facial features and occlusal relationship were recorded. SPSS 23.0 software package was used for statistical analysis of the data. RESULTS: The operation time of 3D group was significantly shorter than that of the control group, and the amount of bleeding was significantly less than that of the control group(P<0.05). The recovery rate of facial appearance and occlusal relationship in 3D group was significantly higher than in the control group(95.24% vs 78.57%, P<0.05). Compared with the control group, the movement distance of mandibular points in 3D group was significantly smaller before and after operation(P<0.05). The satisfaction scores of chewing function and pronunciation recovery in the two groups were close(P>0.05), but compared with the control group, the satisfaction scores of appearance recovery in the 3D group were significantly higher(P<0.05). CONCLUSIONS: 3D reconstruction under 3D printing technology can reduce intraoperative bleeding, shorten the operation duration, and achieve good shape recovery with high degree of satisfaction.
