活髓切断术用于龋源性露髓恒牙的效果评价

Assessment of pulpotomy in cariously exposed permanent teeth

摘要: 目的观察活髓切断术用于牙根发育已完成、龋源性露髓恒牙的临床效果。方法收集龋源性露髓且牙根发育完成的患牙62例,行活髓切断术的患牙为实验组(32例),行根管治疗的患牙为对照组(30例),分别于1、3、6、12个月进行复诊,通过临床及影像学检查评价治疗效果。采用SPSS18.0软件包对数据进行统计学分析。结果观察1 a,活髓切断术的成功率为93.75%,根管治疗的成功率为93.10%,2组成功率无显著差异(χ2=0.010,P=0.665)。结论活髓切断术用于龋源性露髓、牙根发育完成的恒牙,既保留了部分牙髓组织,也避免了牙体组织预备,适合临床推应用。

Abstract: To observe the clinical effect of pulpotomy for cariously exposed permanent teeth with closed apices and provide guidance for clinical application. METHODS: Sixty-two cariously exposed permanent teeth with closed apices and vital pulps were included. Preoperative pulpal and periapical diagnosis was established. The teeth were divided into two groups, the experimental group was treated with pulpotomy(32 cases), and the control group was treated with one-visit root canal therapy (one-visit RCT) (30 cases). Clinical and radiographic evaluation was completed at 1 month, 3 months, 6 months, 12 months postoperatively. Statistical analysis was performed using SPSS 18.0 software package. RESULTS: After 1 year, the success rate of pulpotomy was 93.75%, and the success rate of root canal therapy was 93.10%. There was no significant difference between pulpotomy and root canal therapy( χ2=0.010,P=0.665). CONCLUSIONS: The advantages of pulpotomy are less invasive and less tooth destruction. It is a successful treatment option for cariously exposed pulps in mature permanent teeth.
